The Most Common Equipment Issues We See in Busy Clinics

Calibration Drift: A Silent Problem That Shows Up In Audits

Multiple studies show that calibration drift often occurs slowly and silently, undetected by users until it’s too late. 

Calibration devices usually continue to function, they just become inaccurate. A scale that’s slightly off. A vital signs monitor produces inconsistent readings. An audiometer that no longer matches reference tones. Usually, the problem goes unnoticed until documentation is reviewed or two conflicting patient results raise questions.

Why It Happens

Calibration drift is one of the most documented issues in healthcare equipment management. A 2021 review found that a major root cause of equipment failure is simply the lack of regular calibration and maintenance strategy (Khan et al., PowerTech Journal). Another industry review notes that devices rely on “ongoing performance verification to remain accurate and safe to use” (Belimed, 2022).

If we had to pick one issue that disrupts clinic flow most often, this would be it. 

Rechargeable devices that won’t power on. Vital signs monitors dying mid-exam.
Autoclaves that shut off before finishing a cycle. Handheld tools that only hold a partial charge.

Common autoclave issues include inconsistent temperature or pressure readings, failing seals, slow warm up times, incomplete cycles, drying issues, and failed spore tests. Any of these issues can shut down your sterilization process. Poor maintenance ranks as one of the most common causes of sterilization equipment failure (Belimed, 2022).

Soft parts like blood pressure cuffs, EKG leads, SpO2 sensors, power cords, and thermometer probes fail faster than almost anything else in a clinic.

Not because they’re poorly made but because they’re used hundreds of times a week. Research confirms that wear and tear of soft components is a key contributor to equipment failure, especially when routine inspections are missed (Khan et al., 2021; SoftPro Medical, 2023).

Common Issues We See Often

  • BP cuffs that don’t hold pressure

  • EKG leads with intermittent noise

  • SpO2 sensors that read inconsistently

  • Power cords with exposed wiring

  • Tubing with micro-cracks

  • Thermometer probes that misread

What it looks like operationally

A BP cuff that leaks by 1-2 mmHg doesn’t seem like much - until you’re re-checking every other patient. It slows rooms down, adds stress to your team, and chips away at schedule stability.

  • Replace soft part components more often than you think you need to

  • Run quick functionality checks monthly

  • Include cable and seal inspections in preventive visits

Most of these failures aren’t dramatic, they’re just disruptive. But that disruption adds up and can end up costing your clinic. 

Documentation Gaps That Create Stress During Audits

Sometimes the equipment works perfectly but if you don’t have the documentation to show  when an audit comes around, you run into problems.

Across the industry, several maintenance-management reviews show that poor documentation is one of the biggest causes of compliance issues and operational delays (Micro Precision Calibration, 2022; Belimed, 2022).

A review published in the International Journal of Advanced Technology and Engineering found that proactive maintenance approaches significantly reduce the rate of unexpected equipment failures by addressing wear, calibration, and performance issues early.

Another study on maintenance management found that clinics with structured preventive programs experienced fewer emergency breakdowns, less downtime, and lower long-term repair costs (Khan et al., 2021).
